Duo App Request 

2-factor authentication is required when ePrescribing controlled meds via Epic and logging in remotely from off campus.  DUO (by Cisco) is the app used for this purpose at Methodist Health System.

Once you submit the form below, the Epic analyst team will complete the needed set-up within your Epic account. The IT Security team will then send you 2 text messages with links, which will enable you to download the DUO app and configure it for Methodist Health System.  This process should take 1 to 2 business days, and the links you receive will be valid for 48 hours.
